To get Medicare home health care:

  1. Your doctor must decide that you need medical care in your home, and make a plan for your care at home.
  2. You must need at least one of the following: intermittent (not full time) skilled nursing care, physical therapy or speech language pathology services.
  3. You must be homebound. This means that you are normally unable to leave home and that leaving home requires "considerable & taxing effort".   When you leave home, it must be infrequent, for a short time, to get medical care, or to attend religious services.
  4. The Medicare program must have approved ("Certified") the home health agency caring for you.  Home Health of Montana is certified by Medicare to provide services for you.
